Camper Application

The Camper application consists of two pages (legal size) that contain all the information needed by parents and kids to get them registered and ready to attend summer camp.

Page 1: Keep and refer to this important information.
Parents, please make sure you and your camper have read this and understand it. The Rules and Dress Code will be enforced.

Page 2: Complete, sign and return.
Complete this page in its entirety and mail it to CFBC, along with required deposit. Please note that medical history, insurance information, permissions and signature are required. This application cannot be accepted and processed unless it is complete.

Parents, please make sure you and your child read and understand all of the policiies, rules, dress code, etc. that are outlined in the Camper application and on this website. All of the rules will be reviewed with all campers on the first day of camp and WILL BE ENFORCED. Any child that does not conform to CFBC rules and policies is subject to disciplinary action, including being sent home early.

Any Camper or Staff member who plans to participate in Paintball (if it is offered as an activity during their session) or in the Challenge Course (including the zip line) must complete additional Releases of Liability for these activities. In addition, a nominal fee of $20 (non-refundable) is due for Paintball in order to cover the costs of paint and CO2 gas. Links to the Releases of Liability and Paintball Rules are shown below.

Challenge Course Release of Liability
All participants in the Challenge Course (including zip line) must read, sign and return this form prior to participation. If the participant is under the age of 18 years, a parent or guardian must sign it, as well.

Paintball Waiver and Release of Liability
All participants in Paintball must read, sign and return this form prior to participation. If the participant is under the age of 18 years, a parent or guardian must sign it, also.

Paintball Safety Rules
These safety rules apply to all Paintball activities on the CFBC campus. They will be enforced. Participants (and parents) should read these prior to participation. In addition, safety briefings will be held prior to each Paintball activity.

Paintball General Rules of Play
These rules will be enfoce unless specifically overridden or modified by a particular Paintball activity, in which case the Field Supervisor or Referees will brief the participants. Rules that affect safety will not be overridden or modified for any reason.
