Summer Camp Applications for Volunteer Staff
When a volunteer Staff application is received in the CFBC office, it is recorded and then forwarded to the Summer Camp Coordinators of each week indicated by the applicant. The decision to use or not use any applicant rests with the Coordinator(s) involved and they will be the one(s) to contact the applicant. All volunteers must be active members in churches of Christ.

Background Screening Forms Required for Volunteer Summer Camp Staff
Section 409.175, Florida Statutes, requires that owners, operators, employees and volunteers working in day or residential summer camps to be screened as outlined in Chapter 435, Florida Statutes. It is the responsibility of the summer camp management to know these laws and be sure the camp is in compliance with them.
This screening is a requirement for all CFBC volunteers who will be in direct contact with campers for 20 hours or more per week. Normally, this excludes only kitchen workers. If there is any doubt about what role you will performing or how much time you will spend in direct contact with campers while volunteering for a summer session, please complete the screening forms. If this paperwork is not complete, you will NOT be permitted to serve in any capacity other than as a kitchen worker.
The required screening includes completion of the following downloadable forms. Please complete and mail these to CFBC at least 30 days prior to your first day of service.
Affidavit of Good Moral Character
All new volunteer staff must complete and have this affidavit notarized before service. NOTE: a notarized signature is required.Employment History Check
These employment checks should cover a two-year period preceeding your volunteer service and should exclude periods of unemployment. Have your current or former (within the last two years) employer complete this and mail it to CFBC.Request for Criminal Records Check
The law allows for a check of local records with any law enforecement agency. This can be done through the county Sheriff's Office in whose jurisdiction the person being screened lives. There is not a standard form that is required, however, the one we provide here with work just fine. Complete the top portion, sign it and take it to your local law enforcement agency.
